PGY 1 Rotations

A. General Medical Wards

1. 5 - 6 months.

2. Team consists of 2 SARs and 5 interns and 1-2 CWRU medical students. There is one team on each Firm.

3. Average of every 5th night call for interns on medical floor. Interval varies so that the intern will not be post call in clinic. Back up by in-house supervising resident at night.

4. Average number of admissions is 4, range of 0-5. After 5, the patients are distributed to other team members in the morning.

5. Usual patient load is 4 - 8 patients.

6. Formal teaching rounds 5 times per week at 9:30. Work rounds with the attending weekly.

7. Firm system with residents assigned to one inpatient unit for all ward rotations.

B. Emergency Department

1. One month.

2. Twelve hour shifts, average of 58 hours per week.

3. Exposure to all types of patients in ED setting, including trauma and surgical patients.

4. Attending present 24 hours a day and reviews all cases.

C. Progressive Cardiac Care Unit

1. One month.

2. Four to five PGY-I residents supervised by senior resident, cardiology attending.

3. Every fourth or fifth night call.

4. Daily attending rounds.

D. Ambulatory Medical Clinic

1. One month rotation.

2. No night call.

3. Evaluate urgent care patients with supervising senior resident preceptor. Patients seen are referred from fellow Firm residents for urgent care, non-Internal Medicine services for evaluation of medical problems and preoperative evaluations, ED follow-ups, or self-referral for urgent problems.

4. Daily morning conference of ambulatory lectures, journal club or case conference.

5. Medical subspecialty and non-medical clinics 1-2 per week during month rotation vary from sigmoidoscopy, GYN, ENT, TB, Derm, etc.

E. Continuity Clinic

1. One-half day per week; call not scheduled the day before clinic. Two half days of clinic during elective and ambulatory rotations.

2. Follow own panel of patients.

3. Co-management available from fellow residents.

4. General medicine faculty preceptor.

F. Vacation: four weeks.
