Precision Public Health and Population Health: New Tools for Understanding, New Opportunities for Intervening

In this interactive discussion, we will review:
• Precision Public Health—a new paradigm for population health improvement;
Cleveland & Cuyahoga Health Data Matters and other data visualization tools to assess population health, generate hypotheses about relationships between social determinants of health and health outcomes, and identify potential opportunities for intervention;
• A cutting edge mode proposed for MetroHealth to increase patient engagement with their health ; and
• Opportunities available for clinicians, researchers and health systems through Case Western Reserve University’s engagement with NODEHealth, the Network of Digital Evidence in Health, led by New York’s Mount Sinai Medical Center.
